The Commonwealth of Massachusetts REQUIRES all full-time students to provide documentation of 2 MMR (measles, mumps, rubella) vaccinations, completion of Hepatitis B series, Tetanus within the last 10 years, and meningitis vaccination or signed waiver. All nursing students are required to have a rubella titer and a varicella titer or documented history of varicella or varicella vaccination.
All students attending college must complete the Risk Assessment Questionnaire on page 5 of the Health Form regarding Tuberculosis with their physician. The Health Form (PDF) must be completed in its entirety and returned to Health Services before the start of the semester. You should complete Parts A and B prior to submitting it to your physician. Please bring Part C physical exam and Part D immunizations to your doctor. (Doctor's forms are also acceptable.) The physical examination must have been performed within 12 months of entering Curry.
The Health Form is separate from the forms mailed by other departments, Resident's Life and Athletics. This form must be returned to Health Services. If this form is returned to another department, there is no guarantee it will find its way to our office.
STUDENTS WHO DO NOT SUBMIT A COMPLETE IMMUNIZATION RECORD (PART D) WILL NOT BE ALLOWED TO REGISTER. PLEASE DO NOT PLAN TO ARRIVE ON CAMPUS WITHOUT THE NECESSARY INFORMATION.
Health services is available only to full-time undergraduate students (12 or more credits) who complete the required forms. This information is strictly for the use of health services and will not be released to anyone without your knowledge and written consent.
In addition to the Health Form required by Health Services, Intercollegiate Athletes will be required to send a separate copy of the physical examination PART C to the Athletic Department.
