The Curry College’s Incident Reporting and Risk Management Program provides the guidelines for reporting and reviewing campus incidents. This website provides individuals with the ability to create and submit Incident Reports electronically and through the anonymous hotline. Knowledge of incidents allows the College to identify hazards, gaps in our safety and health programs, and criminal or suspicious activity. More importantly, this knowledge enables us to identify and implement corrective actions to prevent similar incidents in the future. The information below will help you understand the type of incident you are reporting. If you have any questions or need assistance, please call the College’s Office of Public Safety.

Types of Incidents

Accident

An unplanned event that causes damage, injury, or loss. This includes incidents such as slips, trips, falls, equipment-related accidents, and vehicular accidents on campus. It also covers workplace accidents involving employees, including student employees while performing their job duties.

Injury

Any injury suffered by a student, employee, or visitor, whether minor (e.g., cuts or bruises) or serious (e.g., fractures). Injuries resulting from accidents should also be reported.

Near Miss

An incident that could have caused harm or damage but did not. Reporting near misses helps prevent future accidents by identifying risks before they lead to injury.

Laboratory Safety Incident

Any unsafe event occurring in a laboratory setting, including chemical spills, equipment malfunctions, exposure to hazardous materials, or failure to follow safety procedures.

IT and Data Security Incident

Any event that compromises or threatens digital systems or data. Examples include phishing emails, unauthorized access, data breaches, or misuse of College IT resources.

Criminal or Suspicious Activity

Any behavior that may be illegal or poses a threat to campus safety, such as theft, vandalism, assault, drug-related activity, or suspicious individuals or actions.
